摘要:突發(fā)新型冠狀病毒肺炎(COVID-19)已蔓延至全球幾乎所有國家,累計(jì)確診病例已超過1億例。這一全球大流行病盡管現(xiàn)階段在我國多點(diǎn)散發(fā),但已得到基本控制,由于不容樂觀的全球疫情擴(kuò)散,依然存在疫情反彈之巨大風(fēng)險(xiǎn)。在新冠疫苗實(shí)現(xiàn)大規(guī)模接種之前,COVID-19暴發(fā)有效預(yù)警機(jī)制依然是最需要的技術(shù)手段。鑒于感染者糞便/尿液中存在病毒粒子或遺傳物質(zhì)核糖核酸(RNA),荷蘭學(xué)者提出了基于污水流行病學(xué)(WBE)監(jiān)控SARS-CoV-2(誘發(fā)COVID-19的病毒)出現(xiàn)并傳播的設(shè)想。目前,污水中SARS-CoV-2有限RNA檢測研究結(jié)果已經(jīng)證明了這一技術(shù)作為疫情預(yù)警的可行性,亦可能根據(jù)WBE檢測結(jié)果表征病毒傳播動(dòng)態(tài)與趨勢、估算感染群體比例,甚至在一定范圍內(nèi)進(jìn)行感染群體追蹤。在總結(jié)WBE技術(shù)在我國發(fā)展應(yīng)用的基礎(chǔ)上,對(duì)已有污水中SARS-CoV-2的核酸檢測研究結(jié)果、應(yīng)用價(jià)值和指導(dǎo)意義進(jìn)行綜合分析,總結(jié)WBE研究標(biāo)準(zhǔn)化方案,辨析亟需深入研究的方向領(lǐng)域。這對(duì)我國下一階段疫情防控具有相當(dāng)啟發(fā)與參考價(jià)值。
Abstract:The New Corona Virus Disease (COVID-19) has spread to almost all countries in the world with the cumulative number of confirmed cases reaching 100 million cases. Although the virus spread has been under control in China with occasional cases reported at this stage, there is still a huge risk of the epidemic rebound due to the external unfavorable situation. Before the virus vaccination covers a large group of people, an effective early warning system of COVID-19 outbreak is still the most needed technical means. In view of the presence of virus particles or genetic material (RNA) in the feces/urine of infected cases, Dutch researcher firstly proposed the idea of monitoring SARS-CoV-2 via wastewater-based epidemiology (WBE). So far, the preliminary studies have proved the feasibility of this technology on SARS-CoV-2 surveillance as an early warning. WBE may also characterize the virus transmission dynamics and trends, estimate the prevalence of infected cases, and even locate the infected cases. The present review firstly summarized the development and application of WBE technology in China. Then, a comprehensive analysis was conducted regarding so far what we know about WBE and what WBE can do for us in terms of SARS-CoV-2 surveillance. Finally, a standard protocol for WBE implementation and the areas for further research were presented. Overall, this work is expected to convey timely information and be a reference for the further epidemic control in China.
